Annual Declaration Forms

If you currently have a Criminal Background Check (CBC) on file at the office which allows you to volunteer in the school and attend school trips, an Annual Declaration form is being sent home with your child today. Please take the time to fill it out fully and send it back to school prior to September 30th. If you fail to submit the annual declaration, your CBC will be removed from our database and you will be required to go through the process again (which includes paying the fee again).
If you would like to obtain your CBC to participate in school activities these forms can be requested through the main office.
